Care Manager Job Description

 

Summary: The care manager provides assessments, plan development, supportive counseling, on-going monitoring of services, supervision of paraprofessionals, assistance in finding appropriate living arrangements, and information and referral services to elderly and disabled adults and their families. More complex and difficult cases are assigned to masters degree care managers, but the job duties, knowledge and skill requirements are the same for bachelor or master trained care managers.

 

Reports to: Clinical Director Status: Exempt

 

Educational Requirement

A bachelors or masters degree in social work, nursing, or a related field is required and a current license or certification if one is available in the field of study.

 

Experience Requirement

A minimum of 5 years of experience in providing service to the elderly or disabled adults and their families.

 

Job Duties

•  Provide assessments to those who request service from Stowell Associates • SelectStaff.

•  Development, implementation, and evaluation of care management and caregiver care plans for clients.

•  Accurately and completely fill out all forms and documentation required by the agency.

•  Provide supportive counseling and consulting services to clients and families, on-going monitoring and revision of care plans, client advocacy, and communication with families who live at a distance.

•  Provide crisis intervention to clients and families as required, which may include being available nights, weekends or holidays.

•  Maintain confidentiality of all client records and client information.

•  Orient and supervise paraprofessional caregivers in the residence of clients, and provide appropriate feedback to SelectStaff concerning their capacity for quality assurance purposes.

•  Participate in performance review and, if necessary, counseling of caregivers.

•  Complete Comprehensive Evaluations for Protective Placement (CEPP) of clients in a thorough and timely manner, following the required protocol, in guardianship proceedings referred to the agency by attorneys.

•  Provide assessments, benefit determinations and on going care management for clients, in a thorough and timely manner, referred to the agency from the Family Caring Network (FCN) and any other national contract for assessment and care management services the agency may develop, following the required protocol and required schedule for completion.

•  Provide professional coverage in the agency's offices as requested, including answering inquiries for service and providing crisis response as needed for all agency clients if the care manager assigned to the client is not available.

•  Keep accurate, up to date and complete records of the time spent on client and agency business by the sixth business day of the following month.

•  Review and approve billing for time identified to each client in caseload

•  Terminate client files following the appropriate protocol in the time frame required.

•  Attend and participate in weekly care management meetings. A designated care manager will also attend quarterly Utilization Review meetings. Attend supervision meetings, office meetings, performance evaluation meetings, and other meetings as scheduled.

•  Complete necessary continuing education credits each year, and keep state licenses and/or certifications current.

•  Other duties as assigned.

 

Knowledge Requirements

•  Knowledgeable about the aging process, and about the functional challenges posed by disability and/or illness.

•  Familiarity with community resources for the elderly and/or disabled.

•  Specific knowledge and experience with care management, including knowledge of assessment, care plan development, implementation and evaluation of services.

•  Knowledge of conflict resolution and mediation methods.

•  Working knowledge of Microsoft Office Suite, including email.

•  Knowledge of cultural diversity.

Skill Requirements

•  Good writing, interviewing and assessment skills, including ability to accurately and completely document all information required by the agency.

•  Ability to develop creative and individualized care plans that respond to the needs of the client as gathered in the assessment process.

•  Ability to work well with a team approach.

•  Ability to advocate the best needs of the client.

•  Good organizational skills, with the ability to attend to details, while still considering the over-all picture, and prioritizing and handling multiple client demands in a timely fashion.

Caregiver Job Description

 

Summary: Caregivers provide the agency's direct service to its clients. There are three titles that are used to describe caregivers: certified nursing assistants, personal care aides and companions. An employee may have suitable experience to perform only one of these jobs, or she/he may have the appropriate experience and skills to perform all of these jobs. The client may require caregivers who can perform one of these jobs or she/he may require caregivers to perform all of these jobs.

 

Reports to : Care managers and Quality Improvement Manager

Status: Non-exempt

 

Education Requirements

A high school degree or equivalent reading and writing skills; certification as a nurse's aide or proof of required training.

 

Experience Requirements

At least six months of paid work experience in a similar position or verifiable unpaid caregiving experience.

 

Job Duties

 

•  Companion: Provides recreational and leisure time activities to clients, including accompanying clients on trips out of the home for shopping, meals, appointments, and recreational activities. Establishing relationships with clients and families to promote self-respect and encourage client independence when appropriate. On occasion, some caregivers are expected to drive clients to appointments and other events. Performs all tasks associated with household maintenance including cleaning, laundry, bed making, food shopping and meal preparation, dusting, vacuuming, washing floors, and all other tasks needed to keep a house clean, neat, and running properly.

•  Certified Nurse Aide and Personal Care Aide: Provides assistance in all activities of daily living, including bathing, eating, ambulating, dressing, toileting, transferring, and assistance with medication. May also be required to perform Companion duties.

•  All caregivers are responsible for the implementation of the care plan developed for each client.

•  All caregivers are responsible for the preparation of accurate logging and notes as required in the care plan, identifying the tasks completed and providing information about progress on care plan, and any changes (improvements or deterioration of the person served) for the next caregiver, the supervising care manager and/or others concerning what occurred on the shift.

 

Knowledge Requirements

•  Knowledge of operating household appliances including laundry, housekeeping, and kitchen equipment.

•  Knowledge of food preparation, including a basic knowledge of healthy meal planning and how to set a table.

•  Knowledge of appropriate way to assist a disabled person in transferring from bed to chair, chair to walker, in and out of a bath tub or shower, and on and off a toilet.

•  Knowledge of shopping and recreational opportunities in the community.

•  Knowledge of the special needs of the elderly and disabled population served.

Skill Requirements

•  Good communication skills with an ability to listen carefully and understand a person with hearing, vision, or memory loss.

•  Good ability to record daily activity so that other caregivers are aware of what happened on each shift.

•  Good ability to solve problems that arise, including the ability to call the office and/or care manager when a change in a client's status requires professional intervention.

•  Good ability to prioritize and manage a variety of tasks.

•  Good driving skills for the caregivers expected to drive clients.

Personal Requirements

•  Absolute honesty and integrity.

•  Able to remain calm and avoid inappropriate responses in the face of difficult behavior from a client.

•  The ability to recognize when boundaries are not clear and the need to request assistance from the supervising care manager.

•  The ability to maintain confidentiality of healthcare and personal information.

•  For caregivers who drive clients, they must have a current valid driver's license and current vehicle insurance coverage. If client is not present in the automobile, caregivers with a current valid driver's license may perform errands.
